To register at our GP Practice please:

1. Complete a health questionnaire:

• Fill in an online health questionnaire form
• Print a paper copy of the health questionnaire form

Alternatively, our Reception Team can provide you with a copy.

2. Visit either of our surgeries with your photo ID and proof of address (a driving licence will meet both criteria) We also ask that you provide us with a recent blood pressure reading, we have a blood pressure machine located behind a screen in Reception which is free to use at any time.
